New Approaches to Transplantation Symposium Presentation: ‘ONE DAY RENAL TRANSPLANT WORKUP’ – A NEW CARE MODEL
The ’One Day Renal Transplant Workup’ program is an patient centred, equity-focussed quality improvement initiative developed in partnership with Te Pare o Toi by the Nephrology Service of Hauroa o Toi, Bay of Plenty. Its primary aim is to reduce barriers to achieving successful and early transplant listing, and reduce inequities of access for patients with Chronic Kidney Disease, by streamlining and shortening the time taken to complete the renal transplant workup process.
Completing the majority of transplant investigations in ‘one day’, the hope is to increase listing rates, with a strong focus on increasing pre-emptive transplant for Māori and better serving our rural, remote and regional patients.
This presentation will go through the programs development, and talk about future plans to implement this model of care on other centres in New Zealand
